Medical reports/Letters/Form completions/Private Medicals
Some of the requests we have from patients are that they need a letter’s or a form that needs completing etc. If you need a GP to assist you, by writing a letter, or completing a form. Or even a medical report. This work doesn’t come under our General medical services contract and is therefore chargeable work. The said GP will determine if they can carry out this private work on your behalf. And our staff will let you know what the fee is for this work.
Under BMA guidelines we aim to return forms and reports within 21 working days of receipt. In some cases it may take longer if the GP who needs to carry out this work is unavailable.
The Private medicals (HGV license, Taxi Medical) are carried out by Dr M Jheeta. This is usually on a Friday and is determined by his schedule. The fee for this is £120.00 paid in advance. For any DVLA requests re patients, DVLA will pay the fee and This appointment will again be determined by GP Availability. For our letters, fees should be paid in cash to our reception team. A receipt will be provided. Some examples of requests we receive are:

A signed consent form by the patient is required for release of any personal information. Appointments for these services are at the doctor’s discretion. Charges may vary depending on length and detail required in the report, all charges are in line with current BMA guidelines. Excess or multiple requests may incur additional fees.
